There are several FDA approved medications for substance use disorders, including buprenorphine (Suboxone and other brand names), methadone, disulfiram (Antabuse and other brand names), naltrexone (Vivitrol and other brand names) and acamprosate (Camprol and other brand names).

The off-label use of addiction medications is also an important resource, and can include the use of topirimate, gabapentin, baclofen, modafinil and others. Off-label uses may also be pertinent to activity addictions such as gambling, pornography and video games.

We keep naloxone in the office for emergencies, and encourage you to obtain naloxone if you use opiates.

Our medical director is Mario Salguero, MD, Ph.D. Dr. Salguero is available to see you in our offices. We also work as needed with several additional psychiatric and addiction medicine consultants. We will work with your established providers if you wish.

Dr. Salguero, in addition to the medications already mentioned, is experienced with the Sinclair method, and other evidence based novel modalities. He has an ongoing interest and stays current on emerging medication options, both for addiction and psychiatry.

In addition to addiction medications, there are many psychiatric medications to consider. Dr. Salguero takes the time to get to know you. He carefully and collaboratively creates a medication plan that suits you. His initial visit is typically a 60 to 90 minute session, depending on the complexity of your problems.

If you are medically suitable, Dr. Salguero can manage you in outpatient detox (as opposed to entering the hospital or a residential facility).

We have many clients on maintenance buprenorphine. We have clients who see Dr. Salguero only, for maintenance medications, and do not engage in additional psychological services. At Practical Recovery each client, in collaboration with staff, develops and follows her or his own change plan.

A brief professional bio of Dr. Salguero:

Mario Salguero, M.D., Ph.D., graduated from the University of Guadalajara Medical School after which he earned a Diploma of Clinical Nutrition at the National Institute of Nutrition and a Ph.D. in Molecular Biology in Medicine, with a focus on personalized medicine and the clinical applications of genetic polymorphisms and epigenetics. Dr. Salguero completed his Psychiatry Residency at the University of California at San Diego, where he received the “Best Resident Clinician” award three years in a row and the prestigious “Lewis L. Judd” Research Award for his research in implementing a mindfulness self-compassion intervention for residents. Mario has authored articles and chapters in scientific journals and in textbooks and fostered international collaborations, especially around integrative medicine. A Voluntary Assistant Clinical Professor at the Department of Psychiatry, UCSD School of Medicine, Mario supervises residents in training. In addition, he has a Private Practice with a focus in personalizing the relationship with patients, promoting self-healing and integrating treatments from different healing disciplines as evidence supports.
